If you are a new patient to Dalton Surgical
Group, P.C. follow these simple steps to access all of your pre-visit paperwork
and important information you will need before your first visit.
- Visit our "Registration" page and fill out all of the appropriate
fields.
- Once you receive the welcome email, you will need to follow the link
back to our web site to validate your email address.
- Once you have validated your email address, you may begin filling
out the pre-visit paperwork at your convenience.
- After filling out the paperwork, you will need to bring your valid
email account to the office along with two forms of picture I.D.,and
your insurance information.
- Finally we will print your paperwork and will ask for your signature
on the appropriate forms.
If at any time you wish to finish your paperwork at a different time,
all you need is your email address and password to login to the site.
You can find the main login screen by choosing "Login" at the top right
of the page. For your convenience you may also download and print blank
forms to fill out at the convenience of your home after you have registered
